GETTING A BIRTH OR DEATH CERTIFICATE

Birth Certificates 

• The office only issues certificates for births that occurred in Ohio from 1908 – present. 
• For births outside the State of Ohio, please contact the state in which the event occurred. 
• For Birth records in Lawrence county prior to 1908, contact the Lawrence County Probate Court 740-533-4343.  For Birth records in any other county prior to 1908, contact the probate court of that county.

Death Certificates 

• This office only issues certificates for deaths that occurred within the County limits of Lawrence from 1908 – present.
• The address of the death must fall within the County limits of Lawrence.  If you are unsure if the address of the death falls within the County of Belmont you can use this site to confirm the address: Lawrence Property Search  
• For Death records in Lawrence County prior to 1908, contact the Lawrence County Probate Court 740-533-4343.  For Death records in any other county prior to 1908, contact the probate court of that county.
• For deaths outside the State of Ohio, please contact the state in which the event occurred. 
• For deaths occurring outside of the County of Lawrence, but within Ohio, contact the county's office of vital statistics where the death occurred or the Ohio Department of Health.  
• The year of death is mandatory to obtain a Death Certificate.  If you do not know the exact date please enter (99/99/2000).
